HACC Minimum Data Set (MDS2) Reporter

The Home and Community Care (HACC) Minimum Data Set v2 (MDS2) is a collection of data about HACC clients and types of assistance being provided to them through the HACC Program.

We’ve built our own electronic reporting and validation tool that imports clients, cases and activities from CiviCRM (and other data sources) and reports them to HACC.
